Qualifications

You must fill out an application and answer medical questions. Fluid testing may also be required, along with other underwriting requirements.

Teeth cleaning, fillings and preventative care, fluoride protection for kids under 18 years of age, regular check ups, fillings, root canals, extractions, cleanings, x-rays, periodontics, and endodontics. Some plans cover dentures, bridges and orthodontics.
